It is in rule of thirds which draws our attention to certain elements of the picture. Her eyes are in line with the top third. Eyes can suggest a lot and her facial expression is very prominent in this picture, for example the drooped eyebrows and slightly closed eyes suggests sadness and this is combined with her gesture of the hand wiping her nose. When people cry or even are distraught, their nose often runs and she might be wiping away that along with her tears. There has clearly been a direct impact on her and being elderly suggests this could be even more heartbreaking because it might be all she knows.
